I declare that I am over 18 years of age. I agree and hereby accept on behalf of myself and all other persons on this booking form, that this booking is made in accordance with the conditions on this website and to make all payments in relation to these.

I will send a cheque to the address opposite for:

(Payable to "Norfolk Cottages") this being 25% of the total cost of my holiday and I will forward the balance 8 weeks prior to the start of my holiday.

ALL MALE/FEMALE PARTIES
I will send a separate cheque for £100 as a damage/hygiene deposit that I understand to be refunded at the end of the holiday, providing the cottage is found to be in good order.
